                   COVID-19 Workplace Health Screening and Declaration App
The Health E-Passport App is intended to be used as a daily self-declaration health check to promote personal and workplace health adn safety and to stop the spread of the Coronavirus. It also offers users the ability to contact a medical provider to seek medical advice should they need it. This approach is consistent with current Australian public health advice:

PRIVACY AND CONSENT
Reduce the risk of COVID-19 entering the workplace
  The Health E-Passport App is managed by Company Medical Services (CMS). CMS is a provider of medical services. With your consent, CMS will collect information about you and your health for the purpose of promoting workplace health and safety and will remain strictly confidential.
The questions in the daily health declaration aligns with the Australian Department of Health guidelines to help identify symptoms of COVID-19. CMS will only send the status of your health assessment to your emplolyer to indicate the validity of your Health E-Passport.
The details of your health assessment and declaration will remain confidential and will only be shared with your employer with your consent, except where:
- You are diagnosed with a notifiable disease, such as COVID-19 which must by law be reported to governmental authorities
- CMS is otherwise required by law to disclose this information
